Spotlight/blur/highlight tool: One thing I'd love to see is the ability to change the shape of the tool, and/or invert it. The default is a rectangular shape, but often an oval would be ideal. Also, spotlight allows you to select an area, and it darkens everything else. It would be nice if we could darken an area and spotlight everything else. Blur and highlight tools are basically the opposite of spotlight, so the same functionality here would be great. Functionally, you could even merge these three tools, and then just include "Blur", "Transparency" and "Darkness" items in the menu together.

Image callout: I'm not sure if this is a bug. When I use an image for my callout, if I try to change the border, instead of putting a border around the image it recolors all of the borders in my image. I would love to see an option to just put a border around the image.

Some of the shapes allow entering text, and others don't. It's a bit weird that "Filled Rounded Rectangle" and "Rounded Rectangle" are basically the same thing, except that with the latter you can't add fill or text. But, with the former you can choose "no fill" and not add text, and it's the same thing. Why not just take the "rounded rectangle" off the menu, since it's redundant and less useful anyways?

With CS 8, some of these issues have been addressed.

Spotlight/blur/highlight/pixelate: We haven’t made a lot of changes here; it is still limited to a rectangular shape.

Image callout: We added the ability to add a customizable border to images (or any other visual media). There’s also much more flexible control over drop shadows.

Callout shapes: We’ve added some new shapes, removed some of the older less useful ones, and added a new default style.
