Creating Surveys?

Hi! I am creating a course fair video survey for my high school. What I want students to be able to do is
1. enter their name (so we can keep track of responses)
2. watch a short video clip
3. select between 1-3 (in terms of preference for each clip) and then
4. student responses to be recorded/compiled so that we (teachers)
         a. can identify the student
         b. identify their preferences for each of the video clips

Is this at all possible? 

Initially I was trying to build it on Google Sheets, but I'm running into problems with it.

you could certainly create that as a set of quiz questions. Results will get emailed to you once a day. But, google sheets should work fine as well for something that's a survey. Up to you!

I would recommend using google forms. This provides privacy for all since a person only see the data/responses that they entered. If you use google sheets others can see what is entered.
