If you want to eliminate this and all possible future video problems, disable what's on board, toss the 450 and get a "modern video card" that can has 4 display/HDMI ports. With your system, I know $$ not a problem.
But that's a 2008 graphics card with only 16 CUDA cores and 512 MB GDDR3 memory.
It's SO Under-powered it's 35watts of Fan-less design. There's just nothing in the tank left to run anything but the screens.
I've got a good card, not fantastic. NVIDIA GeForce GTX 660
It's 130 watts of fan cooled graphics.
1152 CUDA Cores
1.5GB of GDDR5 memory.
And didn't cost much more than the NVS 450 did in 2008.
I can't promise it will make your troubles go away. But I can assure you it's a under-performing card.
Several people agree that your video cards lack the horse power and ram needed.
Several people have multiple monitor setups and haven't had any issues.
I can only add one more multi monitor system to compare to:
3440x1440 + 3840x2160, and capturing is just fine using a current generation video card.
